Ametropia - A generic name for eye conditions characterized by impaired refraction, e.g. myopia, hyperopia and astigmatism.
Amsler Grid - A test used to detect defects or distortions in the central visual field. It consists of a square subdivided into many hundreds of small squares by horizontal and vertical lines, with a small dot in the center.
Lacrimal gland - The small structure in each eye which produces tears. It’s above the outer corner of the eye, and lacrimal ducts run from the inner corner to the nose. See also punctum and nasolacrimal duct obstruction.
Laser - Although this is used as a word now, it was originally the acronym for Light Amplification by the Stimulated Emission of Radiation. There are many lasers, all man-made for specific purposes. Laser light has a single wavelength (i.e., color), rather than all colors like everyday white lights. Its waves are directional rather than scattered, and each wave is in step with the next one, which vastly increases the power of a laser light.

Overcorrection occurs in the occasional person where healing occurs less vigorously than predicted by the laser computer program, and the planned treatment "overcorrection" ; persists. Retina - (RET-ih-nuh) The inside back surface of the eye, which is light-sensitive, and receives the visual information traveling in on the light waves which enter the eye. It converts the images into nerve impulses which are carried by the optic nerve to the visual center of the brain.
Retinal Detachment - The retina has a layer of nerve tissue, and another layer which contains the blood supply. When these two layers separate, vision is lost at the separated area. It can be restored if the detachment is diagnosed early enough.

An eye condition where the membrane that lies beneath the epithelial cells (surface cells) of the cornea is uneven and traps cells below it which should normally rise above it. It impairs vision, is usually hereditary, and fluctuates in severity over the person's lifetime. It's also called Map-Dot-Fingerprint Dystrophy (a name based on how it looks microscopically), and Epithelial Basement Membrane Dystrophy.

Laser surgery for the eyes involves scraping or lifting the cornea and using laser beams to re-shape the tissue beneath it. This has been proven t be effective for astigmatism, near sightedness and far sightedness.
Eyeliners are meant to define the eyes so they should do just that, not be the focal point of your makeup. Eyeliners come in various colors and forms liquid, pencil etc. Keep the eyeliner natural. Pencils are available in many colors (for the best look stick to black or brown) and can be sharpened for a fine line. For a smudged effect use a sponge tip shadow applicator or an eye shadow brush. When your pencils are too soft refrigerate them. When they are too hard soften them by holding them between your fingers.
To line the eye, start on the lower lid, just beneath the lashes. When you are finished always smudge the line with your sponge. Stay as close to the lash line as possible. On the upper lid, draw a line form the inner corner to the outer corner, thickening the line toward the outer corner. After the age of 40 keep the line in between the lashes.

Convex Lens - Lenses with the opposite curvature to concave lenses. They bulge outwards like a ball and are used to correct hyperopia and presbyopia.
Cornea - (KOR-nee-uh) The front clear part of the eye in front of the pupil and iris. It acts as a lens, refracting light rays as they enter the eye. The eye’s crystalline lens refracts them further to focus them on the retina at the back of the eye. The cornea also allows light to pass out through it, making the iris visible, the eye’s color.
People with cataracts can also benefit from laser surgery for the eyes. Through a specific procedure called phacoemulsification, the cataract is broken into smaller pieces. Afterwards, the pieces are suctioned off via a small cut. Dr. Boothe, minor problems can be in the form of hazy film or blurred vision which can easily be corrected. After the cataract is removed, it does not return.
Laser surgery for eyes has a good number of benefits. Surgery can be done in the doctor's office with the patient leaving for home as soon as the procedure is complete. Focusing power of the eye - The combined action of the cornea and the lens to refract light on to the retina. As light enters the eye, it’s refracted (bent) by the cornea, because of the cornea’s curvature. As the light continues further into the eye, passing through the pupil to reach the lens, it’s refracted more, to a variable degree according to how far away from the eye the object is which is reflecting this light. If it’s down the road a ways, like a distant traffic light, it needs to be refracted less in order to land in focus on the retina. If it’s close up, like the clock on the dashboard, it needs to be refracted more.

Trephine - (truh-FEEN) A surgical tool with serrated edges, used to cut circular pieces. It’s used in a LASEK procedure instead of a microkeratome to cut the corneal flap.
Epikeratome - A surgical tool similar to a microkeratome, used to make the corneal flap in an Epi-LASIK procedure. It has a blunt separator where the microkeratome, used in LASIK procedures, has a very thin and sharp blade. It’s another way of making the corneal flap and does not involve the use of alcohol, as LASEK procedures do. Each way of making the corneal flap has its advantages and disadvantages and consultation with an experienced eye surgeon will determine which would be best for you.

DR WILLIAM A. BOOTHE: Vitreous Fluid - The jelly-like fluid that fills the main part of the eye, between the lens and the retina. It’s clear so as to give light rays an unobstructed path to the retina, and to keep that clarity, it contains phagocytes, cells which ingest and destroy foreign matter.
Vitreous Humor - The transparent gel that fills the space inside the eye behind the lens and in front of the retina.

Phacoemulsification - (fay-koh-ee-mul-sih-fih-K AY-shun) A surgical procedure which breaks up a lens containing a cataract, to make it easier to remove. It uses an incision of about 3 mm, through which the tip of an instrument delivers ultrasonic vibration. The resulting tiny pieces of lens material are removed by suction through that same instrument. A foldable artificial lens can be inserted through that same incision and the incision heals by itself.
This step in the procedure can sometimes cause small blood vessels to burst, resulting in bleeding or subconjunctival hemorrhage into the white (sclera) of the eye, a harmless side effect that resolves within several weeks. Increased suction typically causes a transient dimming of vision in the treated eye. Once the eye is immobilized, the flap is created.
This process is achieved with a mechanical microkeratome using a metal blade, or a femtosecond laser microkeratome (procedure known as IntraLASIK) that creates a series of tiny closely arranged bubbles within the cornea. A hinge is left at one end of this flap. The flap is folded back, revealing the stroma, the middle section of the cornea.
The wavefront system shines its specific laser into the eye briefly, and that light is reflected back from the retina, through the pupil, and on to a wavefront sensor. This is done multiple times. What began as a straight laser beam is now a distorted one, after it has passed through the irregularities of that particular eye. The wavefront system records and measures those distortions and creates a 3-D map of the eye. From this map, a treatment is developed to correct the irregularities and improve vision.

Perioperative - Pertaining to the period of time between being admitted to hospital for surgery to being discharged afterwards. A perioperative nurse is one who cares for the patient throughout that time.
Peripheral Vision - Also called “side vision”. Vision to each side of where the eyes are focused. Direct vision uses the fovea, the center part of the macula, which gives the most detailed vision. Peripheral vision uses the areas of the retina surrounding the macula which give less detail but better night vision. Without peripheral vision, a person would have only tunnel vision, and would be legally blind.
Buttonhole Flap - A LASIK problem where the microkeratome cuts through the top surface of the cornea while creating the corneal flap, while lifting it, or while folding it back. It can be caused by loss of the suction which holds the microkeratome against the cornea, or inadequate suction, or by poor matching of microkeratome to patient. If possible, the flap is replaced, and no further surgery is attempted for several months, after it has healed. Sometimes it can cause scarring which impairs vision, but usually there are no permanent ill effects.
Cataract - An opaque or cloudy area that forms in the eye’s lens, impeding vision. They tend to occur with aging, but can also be caused by trauma. Cataracts are treated by the lens being removed and an artificial lens implanted. See intraocular lens.

Macular Degeneration - A deterioration of specialized cells in the macula of the retina which normally detect light and color to give us sharp central vision. With Macular Degeneration they deteriorate as we age, causing severe vision loss and even blindness after about age 65. There are two kinds:
* Wet (neovascular) – where new little blood vessels grow under the retina, leaking blood and fluid and causing further vision loss
* Dry (non-neovascular) – where new blood vessels do not grow, so that this is a less severe form of the disease; however, it sometimes progresses into the Wet form
Macular Degeneration is not fully understood, but it appears to be related to advancing age, smoking, heredity, high blood pressure, obesity, inactivity, and some drugs. Also called Age-Related Macular Degeneration (ARMD).

DR. WILLIAM BOOTHE - EYE SURGERY: Pachymetry - (puh-KIM-uh-tree) Testing of the cornea’s thickness, done to determine whether the cornea is strong enough for a LASIK procedure. It’s also done for some disorders where the cornea becomes thickened by excess water. The instrument used is a pachymeter.
Penetrating Keratoplasty (PK) - A corneal transplant. A full-thickness, circular piece of the cornea is removed and replaced by a similar piece from a donor. It can restore vision to an eye which had previously been blind.

Legal Blindness - A definition of blindness which enables a person to apply for government disability benefits. It can be either a visual acuity of 20/200 (or worse), with corrective lenses, in the better eye, or tunnel vision in the better eye of 20 degrees in diameter. This level of blindness is severe, but does not necessarily prevent a person from functioning at all.
Corneal Transplant - Surgery to replace the cornea, the clear front area of the eye. Corneal tissue comes from a donor and since the cornea has a small blood supply, there is little risk of rejection, and the new cornea can function very well for years. A corneal transplant can be done to treat Keratoconus, Fuch’s Dystrophy, or damage from a severe infection or injury. It’s a painless outpatient procedure.
It's as if the refraction is "ground in" to the middle layers of the cornea! The results are often dramatic and very rapid, with most patients seeing well enough to drive a car without correction the very next day. However, the best post-operative visual acuity may not be obtained until 2 to 3 weeks, or in some cases, even a few months, after the procedure.

New York, NY (PRWeb) January 14, 2007 -- It was announced on Monday that the boards of two of the major Lasik companies have agreed to merge: American Medical Optics will acquire Intralase for $808 Million. AMO has been in the eye care field for many years furnishing contact lenses, eye pharmaceuticals, as well as cataract instruments.
AMO was spun off from Allergan in 2002 and quickly entered the Lasik field by buying up Visx, the first and major Lasik company. The manufacturer benefits not only from the sale of the laser but also every time the laser is used on an eye - a royalty fee is paid by the doctors. Visx has about 60% of the Lasik market. Now AMO will have the Intralase system that performs the first step of Lasik. AMO also has marketed a microkeratome with which the first step of Lasik was performed, however the "blade" was the principle cause of complications before the all-laser technique.
About five years ago, Dr William Boothe Intralase, a start-up laser company from California put a laser on the market that performed the first step of Lasik in a more precise and safer manner than previous blades. Intralase had a great deal of difficulty getting the surgeons to buy and use the laser.
Coma - A higher order aberration which makes points of light look like comets with blurry tail-like smudges. It can be diagnosed and treated with wavefront-guided LASIK procedures.
Complex Wavefront Retreatment - An off-label use of the excimer laser to do corrective secondary surgery after the original wavefront-guided surgery has left the patient with higher-order aberrations.

As I was driving home last week, I noticed a little boy and his mother by the side of the road. excimer laser - Laser energy produced by several rare gas- halide mixtures. The term excimer comes from the concept of an energized molecule with two identical components or excited dimer (contracted to one word exci-mer). In PRK the term has for practical purposes become synonomous with the argon-fluoride (ArF) gas version. The wave length of an ArF excimer laser is in the far ultraviolet range at 193 nm.
far sighted - A refractive abnormality of the eye requiring a plus (positive or convex) lens for correction. The term originated because people who are far or distance sighted can see at a distance more clearly than they can see objects which are closer. The medical term is hyperopia or hypermetropia.

One may experience increased sensitivity to light and some blurring of vision but these are normal and will clear up in a matter of 3-4 days. Doctors often give eye drops or oral medication to prevent infection and to counter some minor side-effects. Recovery is also quick and the person can resume normal activities after 4-5 days.

regression - A return to the original refractive state. Your eyes are very sensitive organs that need the utmost care. Dr. Boothe, and yet, sometimes things can go wrong with your eyes - disease, age or injury or even heredity factors are just unpredictable so that even with the best of care, our eyes seem to malfunction. Dr. William Boothe with the advent of lasik technology, eye correction with the use of surgery has become more affordable, more convenient and even painless. The accuracy by which eye surgeons have dealt with real eye problems has been remarkable. Patients have been thrilled with results. What a miracle! They can see! There are four different types of eyeglasses available. Reading glasses are worn when working close up. Bifocals are eyeglasses with two points of focus for near vision and distant vision. The top part of bifocal lens is used to correct other eye conditions like myopia or astigmatism while the bottom part is used to correct close up vision. Trifocals have three focusing points; one for near vision, one for immediate vision, and one for distant vision. Progressive addition lenses (PALs) uses different focus points but gradually changes them so that no visual sections are as noticeable as in bifocals and trifocals. Bifocal and monovision contacts are also available. In monovision, one eye is corrected for near vision and the other for distant vision. Dr Boothe Ophthalmologist The laser is programmed prior to each procedure for the ablative "cut" based on the patient's pre-operative refractive error (nearsightedness, farsightedness, and astigmatism). Once the laser ablation is completed, the surgeon gently replaces the corneal surface flap to restore surface integrity of the eye. It's as if the refraction is "ground in" to the middle layers of the cornea! The results are often dramatic and very rapid, with most patients seeing well enough to drive a car without correction the very next day. However, the best post-operative visual acuity may not be obtained until 2 to 3 weeks, or in some cases, even a few months, after the procedure. Some eye diseases can have very serious complications. Dr. Boothe, one example is retinopathy which is a complication of diabetes which can compromise vision. If left on its own, it can cause permanent blindness. Through laser surgery for eyes, leaky capillaries, swelling and any build-up of fluids can be removed. This helps on preserving and restoring normal eyesight for these patients. Dr. Boothe, LASIK is an ambulatory procedure; you walk into the surgery center, have the procedure and walk out again. In fact, the actual surgery usually takes less than five minutes, and you're awake the whole time. Occasionally, the doctor will give a mild oral sedative beforehand. Even though the procedure is relatively quick, this does not minimize the importance of having it performed by a highly skilled surgeon with proper equipment because LASIK is a very delicate procedure. You also should have someone accompany you to drive you back home afterward. Dr. Boothe, most people don't feel pain during LASIK surgery. Your eyes are first anesthetized with special drops. The doctor will have you lie down, then make sure your eye is positioned directly under the laser. (One eye is operated on at a time.) A kind of retainer is placed over your eye to keep your eyelids open — normally, this is not uncomfortable. It has a suction ring that keeps your eye pressurized, which is important in LASIK for allowing the surgeon to cut the corneal flap. The flap is then created with either a microkeratome or with a laser, depending on the surgeon's preference. This step in the procedure can sometimes cause small blood vessels to burst, resulting in bleeding or subconjunctival hemorrhage into the white (sclera) of the eye, a harmless side effect that resolves within several weeks. Increased suction typically causes a transient dimming of vision in the treated eye. Once the eye is immobilized, the flap is created. This process is achieved with a mechanical microkeratome using a metal blade, or a femtosecond laser microkeratome (procedure known as IntraLASIK) that creates a series of tiny closely arranged bubbles within the cornea. A hinge is left at one end of this flap. The flap is folded back, revealing the stroma, the middle section of the cornea.

LASIK DALLAS - Dr William Boothe = Dry eye: Dry eye is characterized by a white or mildly reddened eye combined with a sandy, gritty, dry feeling. Paradoxically, dry eye can sometimes be accompanied by excessive reflex tearing. The dryness itself can stimulate an increase in the production of tears. Dry eye, also known as KCS or keratoconjunctivitis sicca, is very common and is increasing in frequency due to an aging population and increased pollution in the air. Dry eye is more common in females. It is sometimes associated with dry mouth (Sjogren's syndrome) or autoimmune diseases. Other factors thought to contribute to dry eye are some drugs such as antihistamines, antidepressants, or diuretics (water pills). There are many OTC eye drops available for the self-medication of dry eyes. These vary in their formulations, consistency, and preservatives used. Although these lubricants are effective for treating many cases of dry eye, a physician also should be consulted.

Tears are a major protective agent for the eyes. Tears not only wash away dust from the eyes, but also soothe the eyes, provide oxygen and nutrients to the cornea, as well as help defend against eye infections by removing microorganisms that can colonize in the eyes. Tears are of composed of three distinctive layers. The outer lipid section consists of an oily film that counters evaporation and keeps the eye moistened. The middle region is made of mostly water that moisturizes the eyes and some nutrients and proteins that assist in limiting eye infections. The inner coating contains mucous that allows the tear film to spread and reduces evaporation from the eye. Each layer is vital to the health of the eyes and different parts of the eye add to the tears which can lead to any number of possible problems like shortage of nutrients or mucous. It also shows that dry eyes are more likely if there is a depletion of tears due to reduction of generation because of age or by weather issues like windy days which can dry out tears quickly. The importance of tears is also shown by all the over the counter medication that uses eye drops, ointments, or oral capsules designed to either keep the eye moisturized or copy the role of tears.
